Integrity issues close automatically per file

Each integrity issue is tied to the file it was found in, matched by file name. When that file is sent again and processes successfully without the problem, the issue is resolved automatically — so a dashboard of open issues reflects what is still actually wrong.

Partial fixes close only what was fixed. An issue found in several files closes once every one of those files has been re-sent without the problem. A file that fails to process does not count, and its issues stay open. “File processing failed” issues close once that file processes successfully, and “Sync failed” issues are always closed by hand.

Resolving an issue manually works exactly as before.

The issues list and its scorecards now load in a fraction of a second on large catalogues, and paging through the list no longer repeats or skips issues.
